JOB TITLE:      COMMUNITY HEALTH  WORKER II  (CHW 2 )

Union:                                   BCGEU

Classification:                     Community Health Worker II – Grid 8

Responsible To:                 Area Field Supervisor

 

Job Description:     Under the direction of the supervisor, provides home support services to clients such as assisting with the activities of daily living, performing delegated tasks upon approval of the agency Registered Nurse, planning and preparing meals and housekeeping. Home Support services are delivered to the client to promote maximum independence and to help / restore individual and family functioning to prevent the placement of clients away from their home/residence. 

  Job Responsibilities: 

·        Provides personal assistance as outlined in the Agency’s personal assistance guidelines, such as but not limited to, bathing, dressing, oral hygiene, hair care and mobilization

 

·        Provides delegated tasks upon receiving training and approval from the agency Registered Nurse, such as but not limited to, medication assist, medication monitor, catheter, application of creams and ointments, insertion of rectal suppositories, exercise and mobilization routines.

 

·        Plans menus, purchases food and prepares and serves meals, including special diets, gives assistance or hand feeds incapacitated adults or children.

 

·        Performs housekeeping tasks, including cleaning and laundry to maintain the efficient upkeep and healthy environment in the home.

 

·        Observes clients and their environments, and reports unsafe conditions to the Supervisor immediately.

 

·        Observes clients and their environments and reports behaviour, physical and / or cognitive changes and /or changes in living arrangements to the Supervisor involved directly with the client.

 

·        Accompanies clients on public trannsportation on outings in accordance with pre-established care plans.

 

·        Completes and maintains related records and documentation such as communication books and progress reports.

 

·        Provides respite care for families in accordance with pre-established care plans.

 

·        Provides information to families in methods of household management, including housekeeping, meal planning, and provides teaching of parenting skills in relation to infant and childcare.

 

·        Attends in-service and staff development meetings as required.

 

·        Performs other related duties as required.

 

 QUALIFICATIONS: Education: 

·      Provincial Community Health Certificate, Home Support Certificate, Resident Care Attendant Certificate or an educational equivalent recognized by the Employer.

 

·        Minimum Grade 12 education.

 Experience: 

·        Recent related experience in the last twelve months which is reasonable and relevant to the position.

·        Or an equivalent combination of education, training, and experience relevant to the position.  

Personal Skills / Abilities: 

·        Ability to work independently and in cooperation with others

 

·        Ability to observe and recognize changes in clients

 

·        Must have the ability to perform tasks required to maintain/restore the physical, social and emotional health of individuals and families and to improve the quality of life in the home.

 

·        Must have the ability to establish and maintain harmonious relations with clients, families and staff.

 

·        Must be able to speak and understand English competently and to understand written and oral instructions.
