Referrals

Besides calling for service yourself as a private-paying client, you may be referred to our agency through one of several ways:

Your family doctor — when he or she recognizes your need for help in your home and refers you to the Fraser Health Authority / Home Health unit.

Fraser Health Authority / Home Health — after assessing your needs for personal care assistance*

*Referral to Fraser health can be made by you, a relative, or

healthcare provider by calling the Central Intake number at

(604) 953-4965 for those in South Delta, or (604) 953-4950

if you live in North Delta, or the Newton or Guilford areas of

Surrey.

ICBC — if you have been involved in a vehicle accident

WorkSafe
— if you are on a Workers Compensation claim

Veterans’ Affairs
— if you are a veteran and qualify for home support service
