Many of our clients have multiple health, physical and emotional needs. Delta Home Support’s home care team works closely with other health care professionals from Fraser Health, such as Occupational and Physical Therapists, Case Managers, and Home Care Nurses to ensure that these clients receive optimal care safely in the setting of their own home.
