The Integrated Medication Management Program (IMMP) for South Delta supports clients in the safe administration, consistent packaging, and delivery of medications at home.

This program is a result of a partnership between Delta Home Support, Fraser Health’s Home Health, and participating Community Pharmacies A Registered Pharmacist comes into the home to assess a client’s medications, vitamins, and supplements. The Pharmacist collects these to package in blister packs for ease of use.

A specially trained Home Support Worker from our agency can help the client to take their medication as prescribed.

The Pharmacist brings all Physicians and other Health Care Providers into a common “loop” to ensure each knows what the others are prescribing and that no prescribed medications will have adverse interactions with the over-the-counter medicines or supplements a client currently uses.

Those who qualify are clients currently receiving Home Support Services who need help with their medications and are willing to work with one of the participating community pharmacies.
